Curriculum Infusion Module Design Samples
The following forms have been developed by the NDCI staff to assist you in producing a CI module design and integrate it into your class, as well as for evaluation of the project’s progress.

New Faculty Expectations
This form is for New College of Education faculty who attend a Real Life Issues Curriculum Infusion Training. This page lists the expectations for new faculty members. Submit this form along with all of the above to your campus coordinator.

Continuing Faculty Expectations
This form is for Continuing College of Education faculty who continue to attend Real Life Issues Curriculum Infusion Trainings. This page lists expectations for faculty members who continue to participate in the project. Submit this form along with all of the above to your campus coordinator.

Guidelines for Real Life Issues Curriculum Infusion Module Design
This worksheet will help you produce a Real Life Issues Curriculum Infusion module to prepare your students to integrate real life issues into their classes or other work (e.g. counseling) with k-12 students.

Faculty Write-Up Rubric
The Write-up Rubric was developed by the NDCI evaluation team to evaluate faculty write-ups. After submitting your module design your write-up will be evaluated based on this form. Use this form to understand our expectations of your Real Life Issues Curriculum Infusion Module Design.

Faculty Questionnaire
The faculty questionnaire must be completed following the delivery of the prevention module. This questionnaire allows us to learn about your experience integrating real issues prevention into your class. We would like to know the challenges, problems, surprises, and rewards as well as the demographic data.

Student Permission Form
Students attending your curriculum infusion class must be given this form. This permission form gives us the authorization to use their real life issues curriculum infusion lesson plan, resource unit, or another product of this course as a sample on our website. Also, we would like to follow their progress once the student has graduated. Students are not obligated to sign this form.
