Admission Applications

Statement of Goals/Objectives	State your reasons for selecting the program, your academic/professional goals.
Letter of Recommendation	Use to apply for admission into NEIU graduate programs.
Financial Statement	For International applicants.
Sample of Bank Letter	For International applicants.
Registrar's Statement	Use to certify that you are in undergraduate student degree candidate if you haven't graduated when applying for admission.

Financial Award Applications

Form Title	Purpose
Application for Graduate Assistantship	Use to apply for Graduate Assistantship positions, online only.
Application for Graudate Merit Tuition Scholarship	Use to apply for a Graduate Merit Tuition Scholarship, online only.
Application for Scholarship	Use to apply for NEIU Scholarships and Foundation awards.
Merit Tuition Scholarship Acceptance	Use if you have been awarded a Graduate Merit Tuition Scholarship.

Candidacy, Petitions and Graduation Applications

Form Title	Purpose
Request for transfer credit/waiver of course/elective course	See requirements for transfer of credits.
Request for Program Change	Request for an official change of program. All requests require a new Statement of Goals and Objectives and two new Letters of Recommendation specific to the new program.
Request for Extension of Time to Complete Degree	List number of courses to be completed and an action plan.
Candidacy Application Packet	See packet for instructions and required candidacy survey.
Application for Graduation	Please fill out the form and submit it to the Graduate College.
Name/Address Change Form	Please fill out the form to change the name and/or address and submit it to the Graduate College.
