Application process – general information

 

The application process is being refined and revised as we move from a paper-based system to a technology-based system. 

 

Generally, this process begins one year in advance of the first major clinical, though there are variations in some programs.  Watch the CEST Group in NEIUport for application information and deadlines during the first 4-5 weeks of the Fall and Spring semester. 

 

THOSE PLANNING TO ENROLL IN A CLINICAL COURSE FOR THE FIRST TIME attend one of the pre-application meetings.  Multiple dates are scheduled in Fall and Spring and are published in the NEWS section of the CEST Group within NEIUport.  Details about application and placement are provided.  Packets will become available in Beck’s Bookstroe AFTER these meetings are held.  Application forms (for one year ahead) will be announced at the time.

 

THOSE PLANNING TO ENROLL IN A CLINICAL COURSE OR STUDENT TEACHING IN THE UPCMING FALL OR SPRING SEMESETER attend one of the MANDATORY pre-placement meetings that are scheduled each semester.  Multiple dates are scheduled in Fall and Spring and are published in the NEWS section of the CEST Group within NEIUport.  A due date for final placement applications will be announced.  The final application process requires completion of an online data form and the PCAP (Prerequisite Check and Plan Form).  Meeting dates are published in the NEWS section of the CEST Group within NEIUport.  

 


 

 

EDFN Classes Information

EDFN School Sites (These are possible sites for completing observation and service hours.  Contact them directly to check availability.  Use the sample script provided in the Introductory packet provided from the CEST Office and distributed in class. 

Approval letters and documentation forms are obtained from the CEST Office PRIOR to going to the school/site where observation and service hours will be completed.  No EDFN student may begin hours at a school/site BEFORE obtainign this packet.

PRIOR TO RECEIVING THE AUTHORIZATION PACKET FROM THE CEST OFFICE:

  1. Complete the online registration form at http://www.formsite.com/itlc/edfnfall06
  2. Submit TB test results to the Health Serivces Office and request a TB Test report.
  3. Submit to CEST:
    1. TB Test report received from Health Services to the CEST Office
    2. Copy of the email received after completing the online registration form (above)
