Lab Safety Contract : This form is to be completed by all students in each laboratory class every semester. The signed originals are to be retained by instructors, and a copy of the lab safety rules is to be posted in each teaching lab.
Field-trip Conduct Form : This form is to be completed each semester by all students in any class that meets anywhere off-campus. Signed originals are to be retained by the instructor.
