Distinguished Service Award

Know of a Student Life staff member who has gone above and beyond to provide outstanding service? Nominate them for the Distinguished Service Award!
The Distinguished Service Award program is in place to give recognition and appreciation to the A&P, Faculty, and Staff within Student Life who go above and beyond their basic job descriptions by providing superior service to any student, co-worker or community member.
Deadline to submit nominations for the Spring award: April 23, 2010.

Nomination Forms
Nomination Forms can be downloaded here. Please fill out all the information and print the completed form prior to submission. Print and keep a copy for your records. Submit completed forms to Ruth Johnson in the Office of the Dean of Students, B 119.
Guidelines
1. Anyone (Student, A&P, Civil Service, Staff, Community, etc.) can nominate a Student Life A&P, Faculty, or Staff representative (excluding Extra Help and Student Workers).
2. Nominee must be from a division of Student life:
a. Office of Student Life b. Office of the Dean of Students c. Office of Career Services d. Student Activities e. Counseling f. Health Services g. Campus Recreation h. El Centro – Student Life Staff Only i. CCICS – Student Life Staff Only
3. Nominee must have worked for Student Life for at least one year.
4. Nomination Forms will be accepted throughout the semester and presented to the Review Committee at the end of each semester. The submission schedule is as follows:

A Review Committee consisting of students, non Student Life faculty and/or staff and one PDC member will meet at the end of each semester to select one nominee to receive the Distinguished Service Award.
A certificate and other rewards will be presented to the chosen nominee at the beginning of the following semester. A brief biography of the winner as well as a distinguished service story about the winner will be posted online on NEIUport as well as on campus for people to view during the following semester.
The award winners for every semester will be recognized at the Student Life Recognition Reception at the end of that fiscal year.
