Veterans Recertification Form
This form is used every semester to verify and recertify veteran benefits.
IVG Request Form
This form is submitted to authorize the use of your IVG. It will remain in effect until you submit the form to terminate.
Request for Change of Program or Place of Training
This form must be submitted by all new freshmen or transfers, or returning veterans who have not attended Northeastern Illinois University for one year or more.
