306A Form
Prior to authorization for 306A, the student must obtain three RECOMMENDATION FORMS from NEIU ELED/BLBC faculty members.
The first recommendation must be from your ELED/BLBC 301 instructor. The second must be from your BLBC/ELED302 instructor. The third recommendation must be from the instructor of record from any ONE of the following: 304 or 305 or 310.
Without these three recommendations you will NOT be placed in a school to complete your Clinical Observation Hours as required by the State of Illinois. An unsatisfactory recommendation may delay or prevent your taking 306A. It is the student's responsibility to give this form to the appropriate faculty member for evaluation during the semester of registration for these courses.
