Prerequisite Verification Forms
Download and print the Prerequisite Verification Form by clicking here (pdf format)
- Fill out the permit form completely, including student ID and phone number.
- Completed forms may be dropped off at SCI 214A.

Note: Permits for 090, 091 and 092 are issued by the Math Development Office. Call 442-5766 or visit SCI 204B for more information.
